Appointment

The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms so that you can complete them in the comfort of your home, at your convenience. Returning the forms by mail allows us the opportunity to enter your information prior to your appointment and helps to determine if additional information is necessary. If, for some reason, you are unable to complete and return the forms prior to your visit, please plan an arrival of ten minutes prior to your scheduled appointment. 

Financial Information

Payment is due at the time of service. Cash, checks, debit and credit cards are accepted. We also participate with Care Credit.  Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss the possibility of  financial arrangements.

Insurance Information

Filing your own insurance can be a time consuming and frustrating process. We are happy to file your dental insurance as a courtesy. Submission does, however, require that we have all insurance information in place at the time of service. Please remember that each employer based plan is negotiated by your Human Resource department and specific to that employer. Each insurance company may have thousands of plans and though we are familiar with many, it is difficult, if not impossible, to determine benefits for each specific plan. We are not responsible nor associated in any way with the processing of your claims other than basic submission. We can, with complete insurance information, typically provide assistance with estimating your portion of your treatment costs and do expect that portion at the time of service. Any residual amount, after receipt of insurance payment is due within 30 days. It is, however, ultimately your responsibility to know your insurance benefits. If you are unable to provide us with complete and accurate insurance information, full payment will be due at time of service and we will provide you with a superbill for your submission to your insurance company.
